Our Services

Eligibility 合格者標準

  • Age 18 or above

  • Medi-Cal holders free of charge

  • Enrollment into a Managed Care Health Plan

  • Have one or more chronic or sub-acute medical conditions including cognitive, mental healthand physical health problems
